Who should not get Botox treatment?

A physician reviews a form with a man across a consultation desk.

Before anyone talks about price, a consent form should say who is left out. Ask it the way a reader in Ann Arbor, Michigan would put it to the front desk: who should not get Botox treatment, and does my paperwork list them?

What the published exclusions say

dermnetnz.org states: "Botulinum-A is contraindicated in individuals with: Pre-existing medical conditions that cause muscle weakness, including motor neurone disease (amyotrophic lateral sclerosis) and myasthenia gravis Infection overlying the injection site Previous allergic or hypersensitive reactions to botulinum-A." medlineplus.gov states: "You should not use Botox if you are pregnant or breastfeeding."

Reading the dermnetnz.org sentence line by line

That one sentence covers three groups. The first is pre-existing medical conditions that cause muscle weakness, with motor neurone disease and myasthenia gravis named. The second is infection overlying the injection site. The third is previous allergic or hypersensitive reactions to botulinum-A, and the medlineplus.gov line adds pregnancy and breastfeeding.

Hold your intake form against that list

A health questionnaire that asks only about allergies leaves the other items unasked. Go through your form and see which of the groups above it actually asks about. Anything it skips is a question to raise out loud, and to have noted in your file.

Why the product name belongs on the paperwork

The MedlinePlus page on Botox and the botulinum toxin entry at dermnetnz.org both describe the treatment in general terms. A PMC review of botulinum toxin type A products carries a title saying those products are not interchangeable, which is a reason to have the exact product named on your form.

The line to ask for in writing

Ask for this sentence: "List in writing every condition that would stop treatment today, and name the product you plan to use." If the clinic cannot produce it, you have learned something about its paperwork.
